MultiCare MN is hiring a full-time Case Manager to join our Housing and Stabilizing Services (HSS) team. This position plays a vital role in supporting individuals and families as they work toward stable housing and improved quality of life.

No prior experience in case management is required – we will provide full training. If you are compassionate, motivated, and interested in making a difference, we encourage you to apply!

Job Responsibilities:

• Assist clients in finding and maintaining stable housing

• Connect clients with community services and resources

• Maintain clear and accurate case documentation

• Work collaboratively with clients, landlords, and service providers

• Support clients in achieving long-term stability and self-sufficiency

Qualifications:

• No experience required in the same field – we provide full training

• Strong communication and organizational skills

• A passion for helping others

• Ability to work independently and as part of a team
